Research questions and backgroundThis study explores a highly controversial issue of medical care in Germany: the decision to withhold or withdraw mechanical ventilation in critically ill patients. It analyzes difficulties in making these decisions and the physicians’ uncertainty in understanding the German terminology of Sterbehilfe, which is used in the context of treatment limitation. Used in everyday language, the word Sterbehilfe carries connotations such as helping the patient in the dying process or helping the patient to enter the dying process. (...) Yet, in the legal and ethical discourse Sterbehilfe indicates several concepts: (1) treatment limitation, i.e., withholding or withdrawing life-sustaining treatment (passive Sterbehilfe), (2) the use of medication for symptom control while taking into account the risk of hastening the patient’s death (indirekte Sterbehilfe), and (3) measures to deliberately terminate the patient’s life (aktive Sterbehilfe). The terminology of Sterbehilfe has been criticized for being too complex and misleading, particularly for practical purposes. Materials and methods An exploratory study based on qualitative interviews was conducted with 28 physicians from nine medical intensive care units in tertiary care hospitals in the German federal state of Baden-Wuerttemberg. The method of data collection was a problem-centered, semi-structured interview using two authentic clinical case examples. In order to shed light on the relation between the physicians’ concepts and the ethical and legal frames of reference, we analyzed their way of using the terms passive and aktive Sterbehilfe. Results Generally, the physicians were more hesitant in making decisions to withdraw rather than withhold mechanical ventilation. Almost half of them assumed a categorical prohibition to withdraw any mechanical ventilation and more than one third felt that treatment ought not to be withdrawn at all. Physicians showed specific uncertainty about classifying the withdrawal of mechanical ventilation as passive Sterbehilfe, and had difficulties understanding that terminating ventilation is not basically illegal, but the permissibility of withdrawal depends on the situation. Conclusions The physicians’ knowledge and skills in interpreting clinical ethical dilemmas require specific improvement on the one hand; on the other hand, the terms passive and aktive Sterbehilfe are less clear than desirable and not as easy to use in clinical practice. Fear of making unjustified or illegal decisions may motivate physicians to continue (even futile) treatment. Physicians strongly opt for more open discussion about end-of-life care to allow for discontinuation of futile treatment and to reduce conflict. (shrink)

Current theory-of-mind research faces the challenge of reconciling two sets of seemingly incompatible findings: Whereas children come to solve explicit verbal false belief tasks from around 4years of age, recent studies with various less explicit measures such as looking time, anticipatory looking, and spontaneous behavior suggest that even infants can succeed on some FB tasks. In response to this tension, two-systems theories propose to distinguish between an early-developing system, tracking simple forms of mental states, and a later-developing system, based on (...) fully developed concepts of belief and other propositional attitudes. One prediction of such theories is that the early-developing system has signature limits concerning aspectuality. We tested this prediction in two experiments. The first experiment showed that 2- and 3-year-olds take into account a protagonist's true or false belief about the location of an object in their active helping behavior. In contrast, toddlers' helping behavior did not differentiate between true and false belief conditions when the protagonist's belief essentially involved aspectuality. Experiment 2 replicated these findings with a more stringent method designed to rule out more parsimonious explanations. Taken together, the current findings are compatible with the possibility that early theory-of-mind reasoning is subject to signature limits as predicted by the two-systems account. (shrink)

Talk by members of executive hospital boards influences the organizational positioning of nurses. Talk is a relational leadership practice. Using a qualitative‐interpretive design we organized focus group meetings wherein members of executive hospital boards (7), nurses (14), physicians (7), and managers (6), from 15 Dutch hospitals, discussed the organizational positioning of nursing during COVID crisis. We found that members of executive hospital boards consider the positioning of nursing in crisis a task of nurses themselves and not as a collective, interdependent, (...) and/or specific board responsibility. Furthermore, members of executive hospital boards talk about the nursing profession as (1) more practical than strategic, (2) ambiguous in positioning, and (3) distinctive from the medical profession. Such talk seemingly contrasts with the notion of interdependence that highlights how actors depend on each other in interaction. Interdependence is central to collaboration in hospital crises. In this paper, therefore, we depart from the members of executive hospital boards as leader and “positioner,” and focus on talk—as a discursive leadership practice—to illuminate leadership and governance in hospitals in crisis, as social, interdependent processes. The number of traumatic events that occur worldwide is increasing, yet the literature pays little attention to their implications for leader development. This paper calls for a consideration of how collective trauma such as genocides and the Holocaust can shape the cognition of leaders who are second- and third-generation descendants. Drawing on research on the transgenerational transmission of collective trauma, social learning, social identity and psychodynamic theories, we identify three mechanisms through which collective trauma can be transmitted to leaders: cultural (...) artifacts, community events and family narratives. In this article, we introduce the topic of collective trauma in leader development and draw on the theories of psychodynamic, social identity and social learning to explain how the transmission of collective trauma occurs. We also propose how the transmitted trauma shapes leader cognition, values and behaviors. This framework can serve as a basis for future theory-building in leader and leadership development that will be key to the study of authentic and ethical leadership. (shrink)

Nous abordons ici l’étude de la phénoménologie des objets visuels à partir de comptes rendus verbaux, de réponses à des questions ou de descriptions spontanées, ainsi que d’associations libres. Nous demandons même parfois aux sujets de réaliser de simples croquis. Cet éventail de méthodes permet de sonder la structure profonde de la conscience visuelle. Celle-ci est avant tout révélée par ce qui n’est pas spontanément mentionné par des qualités accidentelles ou encore par des changements induits lors de variations ou ajouts (...) mineurs. Nous avons trouvé une concordance remarquable parmi un grand nombre de participants. Ceci démonte que les comptes-rendus verbaux permettent d’explorer puissamment la structure de la conscience visuelle. Tout comme le langage, les « objets visuels simples » ont des racines profondes dans la conscience. Ces fondations « invisibles » servent à définir des figures simples, un « carré » par exemple, comme des objets prototypiques et uniques. La nature des « objets visuels » implique ainsi l’existence d’un « vaste domaine invisible ». Sans une telle prise en compte, la compréhension correcte de la nature de l’objet visuel n’est pas possible. Cette découverte devrait permettre de mener à une théorie de la syntaxe formelle des formes visuelles fondamentales. (shrink)

Public announcement logic is an extension of multiagent epistemic logic with dynamic operators to model the informational consequences of announcements to the entire group of agents. We propose an extension of public announcement logic with a dynamic modal operator that expresses what is true after any announcement: after which , does it hold that Kφ? We give various semantic results and show completeness for a Hilbert-style axiomatization of this logic. There is a natural generalization to a logic for arbitrary events.

At the beginning of 2020, COVID-19 became a global problem. Despite all the efforts to emphasize the relevance of preventive measures, not everyone adhered to them. Thus, learning more about the characteristics determining attitudinal and behavioral responses to the pandemic is crucial to improving future interventions. In this study, we applied machine learning on the multi-national data collected by the International Collaboration on the Social and Moral Psychology of COVID-19 (N = 51,404) to test the predictive efficacy of constructs from (...) social, moral, cognitive, and personality psychology, as well as socio-demographic factors, in the attitudinal and behavioral responses to the pandemic. The results point to several valuable insights. Internalized moral identity provided the most consistent predictive contribution—individuals perceiving moral traits as central to their self-concept reported higher adherence to preventive measures. Similar was found for morality as cooperation, symbolized moral identity, self-control, open-mindedness, collective narcissism, while the inverse relationship was evident for the endorsement of conspiracy theories. However, we also found a non-negligible variability in the explained variance and predictive contributions with respect to macro-level factors such as the pandemic stage or cultural region. Overall, the results underscore the importance of morality-related and contextual factors in understanding adherence to public health recommendations during the pandemic. (shrink)

We study the extension of public announcement logic PAL by public assignments, which we call PALA. Just as in the case of PAL, the standard procedure for deciding PALA validity, i.e. the use of so-called reduction axioms to translate PALA formulae into formulae in epistemic logic EL, may lead to exponential growth. In this paper, we show that such a price is not mandatory, for we provide a polynomial translation of PALA into EL. This is based on abbreviations of subformulae (...) by new propositional letters. Such optimal translation also enables us to show the computational complexity of the problem of deciding PALA validity, which turns out to be coNP-complete in the single-agent case and PSPACE-complete in the multiagent case. Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

Sublimation is usually defined as a defense-mechanism that desexualizes the sexual instincts. This desexualization then results in socio-cultural activities and psychic health. That means that sublimation is a crucial concept for psychoanalytic thinking, because it seems to connect the Freudian metapsychology with both applied psychoanalysis and psychoanalytic therapy. However, in this article I argue that within Freud's theory sublimation is an empty and redundant concept. It is a redundant concept as far as it 'explains' the socio-cultural tendencies of human beings, (...) since these tendencies can be easily understood as the outcome of other defense-mechanisms, such as repression and identification. For that matter, Freud's only example of the sublimation process — found in Leonardo da Vinci and a Memory of his Childhood — is in fact no sublimation, but a classic example of repression followed by symptom-formation. Secondly 'sublimation' is an empty concept with regard to the psychoanalytic therapy, because in that context it is used to explain something that simply never happens, at least according to Freud, namely a perfect recovery and a completely 'terminable' analysis. Freud's anthropology is in fact little more than an amplified psychiatry. For Freud, the human being is in essence a sick animal. In this paper the author discusses why Freud made this so-called 'anthropological turn'. First it is shown that Freud wanted his psychoanalytic theory to be a 'Philosophy of Man'. Secondly it is argued that this can only be the case if the determinants of pathology, that psychoanalysis claimed to have discovered, are constitutive of human subjectivity. This means that (...) the defensemechanisms and the partial sexual instincts interact in each of us and do so necessarily. Although Freud usually merely postulates the necessary character of this interaction, which always produces symptoms, one can nevertheless find some argumentation for this relation distributed in his work. The third part of this paper consists in an enumeration and explanation of the reasons Freud gives for the fact that we can never satisfy our sexual instincts and always have to rely on the substitutes he calls symptoms. Some of these reasons can be led back to the conflict between sexuality and culture, but most of them are determined in an organic way. If this Freudian intuition is true, the 'Philosophy of Man' should be replaced by the 'Philosophy of Psychopathology'. (shrink)

These notes contain an annotated edition of the only four extant letters of Burchard de Volder to Philipp van Limborch. In the first letter De Volder provides Van Limborch with some information about the subscription to the Dordrecht Confession of Faith by professors. In the second letter De Volder comments upon Van Limborch’s De veritate religionis Christianae. This letter is interesting as it provides insights into De Volder’s views on religion and theology. The third letter served as a cover letter (...) for De Volder’s sending to Van Limborch a copy of the honestum testimonium on Jacobus Arminius that was requested by Arminius’s widow in 1611. In the fourth letter Volder comments upon the visit that Pieter Burman had paid him. The visit was an episode in the quarrel between Pieter Burman, his brother Frans Burman jr., and Van Limborch, caused by Van Limborch’s remark, in his Theologia Christiana, that Frans Burman sr. had used Spinoza’s words while treating the issue of divine omnipotence in his Synopsis theologiae. La publicación de Speculum: de l’autre femme en 1974, segunda tesis doctoral de la filósofa, psicoanalista y lingüista Luce Irigaray, produjo un revuelo inusitado, expresado tanto en los modos en que se recepcionó la obra, como en la trayectoria de la propia autora. Sus resonancias, presentes aún hoy, están relacionadas con la contundencia de su espíritu crítico, con el estilo lúdico y a momentos hermético de su escritura, y con la profundidad filosófica de los interlocutores que Irigaray convoca para que (...) se presenten en esta inusual escena de experimentación escritural. En varias ocasiones, la autora ha señalado que su trayectoria de pensamiento responde a la dinámica de elementos críticos y constructivos, que se van entrelazando en una especie de continuum. Si seguimos esta clave de lectura, Speculum puede ser leído como un ejercicio de pensamiento que se da en el trasfondo de una tensión entre, por una parte, las intervenciones críticas en el canon filosófico occidental y el discurso psicoanalítico, y por otra, la necesidad de pensar las condiciones para transformar toda una cultura. En lo que sigue, quisiera explorar esta clave de lectura y evaluar sus alcances y posibilidades, proponiendo algunas herramientas conceptuales que se irán presentando a lo largo de la exposición. Johann Christoph Sturm’s natural philosophy, with which Leibniz engages in “De ipsa natura”, as well as Petrus van Musschenbroek’s epistemology, constitute important steps in the process of the speciation of physics. In this case, speciation is understood as the process through which the explanation of natural phenomena via empirical regularities comes to define the whole domain of the newly established niche of physics, to the exclusion both of teleology and efficient causality.

The present book is devoted to the question of what the goal of physics is. The essential result of this is the rejection of traditional proposals for such a goal in favour of a new proposal. In both the rejection of the older proposal for a goal and the endorsement of the new one, I rely on a common practice in physics - the practice of idealisation. Traditional proposals for goals must be abandoned if they cannot explain this practice; my (...) own proposal is supported by the fact that it can be shown to explain the practice of idealisation. There is a close connection between the goal of physics and some positions in the philosophy of science. van Fraassen has drawn attention to this in his The Scientific Image. If such a connection between the question of the goal and various positions in the philosophy of science exists, the result of the investigation into the goal of physics can be used to discuss positions such as empiricism, constructivism and scientific realism. This programme will be carried out in the following steps. Chapter 1 will first clarify in what sense physics can be said to have a goal. Criteria will be worked out, which an adequate explication of the goal of physics should satisfy. It will be shown that candidates for the goal of physics should be able to explain why typical actions essential to physics are carried out in the way that characterises them: They must be recognisable as means to a proposed end. In addition, various proposals for the goal of physics will be presented, and the relationship of these proposals to the philosophy of science positions of empiricism, constructivism and scientific realism will be examined. In Chapter 2, for selected, constitutive modes of action in physics - namely for idealisations – it will be investigated to what extent they can be explained by the proposed target candidates. It will be shown, firstly, that the traditional formulations of the goal of physics with respect to a particular class of of idealisations have only limited explanatory value, and secondly, that the goal of a unified description of physical systems description of physical systems makes this practice understandable. In Chapter 3 it will be shown that a second class of idealising measures - especially abstractions, but also certain experimental procedures - also cannot be made comprehensible by traditional goal ascriptions. Moreover, they cannot be integrated into a picture of theory construction which presupposes the classical, empiricist concept of the law of nature as the as the basis for an understanding of physics. I will argue that it makes sense to define natural laws as the ascription of as the attribution of dispositions to physical systems, i. e. systems, i.e. as statements about the way in which physical systems systems behave under very specific circumstances, namely when they are isolated. Such a conception allows us to present a candidate target that is able to explain all of the idealisation measures under investigation. Thus, it satisfies the condition that a candidate for the goal of physics must fulfil. The goal of physics, it will be asserted, is, is to give a unified description of the dispositions of physical systems. Now, since the traditional proposed goals, which turn out to be inadequate, are closely related to scientific realism, empiricism and constructivism, a re-evaluation of these positions becomes necessary. Society has already seen multiple potential futures of neurotechnologies through the multifaceted lens of science fiction. We do not know which of those futures we will come to live, but the strong force in society's evolutionary process will likely continue to be the insight and oversight developed by the bioethics community, pushing and being pushed by the advances of technology, improvements in medical care, and market competitiveness.

Direct-to-consumer advertising of healthcare products refers to a variety of marketing practices based on a combination of information and promotion strategies directed at consumers through different media such as radio and television broadcasts, newspaper and magazine ads, and, more recently, through the Internet. The principal form of marketing used by the pharmaceutical industry is the distribution of free samples to physicians but DTCA is an increasing part of global promotional spending for prescription drugs. Latest estimates suggest that DTCA now represents (...) an annual $3.2 billion enterprise for the U.S. pharmaceutical industry. Findings from the literature show that these substantial efforts are geared toward the newer pharmaceuticals for chronic conditions with huge market potentials. The subject of musical emotions has emerged only recently as a major area of research. While much work in this area offers fascinating insights to musicological research, assumptions about the nature of emotional experience seem to remain committed to appraisal, representations, and a rule-based or information-processing model of cognition. Over the past three decades alternative ‘embodied’ and ‘enactive’ models of mind have challenged this approach by emphasising the self-organising aspects of cognition, often describing it as an ongoing process of dynamic (...) interactivity between an organism and its environment. More recently, this perspective has been applied to the study of emotion in general, opening up interesting new possibilities for theory and research. This new approach, however, has received rather limited attention in musical contexts. With this in mind, we critically review the history of music and emotion studies, arguing that many existing theories offer only limited views of what musical-emotional experience entails. We then attempt to provide preliminary grounding for an alternative perspective on music and emotion based on the enactive/dynamic systems approach to the study of mind. (shrink)

Alexithymia, psychological distress, and posttraumatic stress disorder (PTSD) are highly related constructs. The ongoing debate about the nature and relationship between these constructs is perpetuated by an overreliance on cross-sectional research. We examined the longitudinal interactive relationship between alexithymia, psychological distress, and PTSD. We hypothesised that there is an interactive relationship between the three constructs. Military personnel (N = 1871) completed the Toronto Alexithymia Scale, the Kessler 10 and a PTSD Checklist (PCL-C) at pre-deployment, post-deployment, and at 3–4 years following (...) the post-deployment assessment. We initially tested whether psychological distress is either a moderator or mediator in the relationship between alexithymia and PTSD across the time points. General psychological distress was a partial mediator of total PTSD severity and hyperarousal symptomology at all three time points. Psychological distress fully mediated re-experiencing and avoidance symptomology at all three time points. Our results suggest that those with alexithymia are at longitudinal risk of developing more severe PTSD symptomology and experiencing hyperarousal irrespective of temporal proximity to traumatic exposure. Further, vulnerability to the emergence of re-experiencing and avoidance symptomology for those with alexithymia is increased when one experiences greater distress. Our results show that alexithymia is a persistent risk factor for PTSD symptomology. (shrink)

A brain-computer interface technology that can decode the neural signals associated with attempted but unarticulated speech could offer a future efficient means of communication for people with severe motor impairments. Recent demonstrations have validated this approach. Here we assume that it will be possible in future to decode imagined (i.e., attempted but unarticulated) speech in people with severe motor impairments, and we consider the characteristics that could maximize the social utility of a BCI for communication. As a social interaction, communication (...) involves the needs and goals of both speaker and listener, particularly in contexts that have significant potential consequences. We explore three high-consequence legal situations in which neurally-decoded speech could have implications:Testimony, where decoded speech is used as evidence;Consent and Capacity, where it may be used as a means of agency and participation such as consent to medical treatment; andHarm, where such communications may be networked or may cause harm to others. We then illustrate how design choices might impact the social and legal acceptability of these technologies. (shrink)

It is not uncommon for patients with advanced disease to express a wish to hasten death. Qualitative studies of the WTHD have found that such a wish may have different meanings, none of which can be understood outside of the patient’s personal and sociocultural background, or which necessarily imply taking concrete steps to ending one’s life. The starting point for the present study was a previous systematic review of qualitative studies of the WTHD in advanced patients. Here we analyse in (...) greater detail the statements made by patients included in that review in order to examine their moral understandings and representations of illness, the dying process and death. We identify and discuss four classes of assumptions: assumptions related to patients’ moral understandings in terms of dignity, autonomy and authenticity; assumptions related to social interactions; assumptions related to the value of life; and assumptions related to medicalisation as an overarching context within which the WTHD is expressed. Our analysis shows how a philosophical perspective can add to an understanding of the WTHD by taking into account cultural and anthropological aspects of the phenomenon. We conclude that the knowledge gained through exploring patients’ experience and moral understandings in the end-of-life context may serve as the basis for care plans and interventions that can help them experience their final days as a meaningful period of life, restoring some sense of personal dignity in those patients who feel this has been lost. The case concerned euthanasia on the basis of an advance euthanasia directive for a patient with severe dementia. In this paper we describe the review process for euthanasia cases in the Netherlands. Then we describe the case in detail, the judgement of the Regional Review Committees (...) for Termination of Life on Request and Euthanasia and the judgement of the medical disciplinary court. Both the review committees and the disciplinary court came to the conclusion there were concerns with this case, which mainly hinged on the wording of the AED. They also addressed the lack of communication with the patient, the absence of oral confirmation of the wish to die and the fact that the euthanasia was performed without the patient being aware of this. However, the doctor was acquitted by the criminal court as the court found she had in fact met all due care criteria laid down in the act. We then describe what this judgement means for euthanasia in the Netherlands. It clarifies the power and reach of AEDs, it allows taking conversations with physicians and the testimony of the family into account when interpreting the AED. However, as a practical consequence the prosecution of this physician has led to fear among doctors about prosecution after euthanasia. (shrink)
